We are seeking more responses to the consent forms for counseling/mental health services, health room services, and participation in clubs. If you have not done so, please complete these three forms for each of your children.

Beginning September 1, 2025, our district must have your consent in the following areas:

1. Mental Health-Related Services

   The district must notify parents of staff services related to school counseling and give parents the opportunity to opt out before those services are provided to their child. Questions? 2. Physical Health-Related Services

   Certain school-based health services, such as non-emergency medical screenings or treatments, now require your written permission. Questions? Reach out to Nurse Bass Cassidy.Bass@austinisd.org LINK to form

    

3. Participation in school-sponsored Student Clubs

   If your child wishes to participate in school or district-sponsored clubs or organizations, parental approval is required. Examples of clubs, organizations, teams, and groups include: Run Club, Garden and Beautification Club, Morning Announcements, Cardinal Cast, etc. Questions?
